We usually use "family doctor" or "family physician," and I think they both work for this. :)

Another option, at least used here in Uk would be:
GP, which stands for General Practitioner.

In my country there are not many "médicos de cabecera" left. Those were the ones who used to go to the house of the patients to examine them and prescribe medication, and would not transfer the patients to the hospital unless it was absolutely necessary . If that is the doctor you are referring to here, then the term family doctor seems suitable

In The USA "medico de cabecera" would be "Primary care doctor" , "Primary care physician" or " Primary care provider".
