Payroll, physiotherapy, medicine, insurance abbreviations

Hi! I'm literally struggling to translate a few acronyms and abbreviations that I have found in a number of documents related to a compensation claim (they belong to different fields of knowledge).

Payroll:

This period totals
NI (National Insurance): A 126.72 (A= average???)

Physiotherapy:

(ankle injury; phys. report)
r/v l/52 (r and l probably stand for right and left; what the rest means is unknown to me!; 52 does appear before: "HPC/5/52 hurt right ankle", "The elactoplaster for 2/52").

UTA
DNA
(both of them appear right before the name of the physiotherapist that has prepared the report; the same place is taken up by "treatment" in the following paragraph).

Medicine:

PAst medical history
B12 deficiency, intrinsic factor antibodies (+ve)
