Question Help w/mechanic communication

I've found this site hoping that it will help me communicate better with my employees when there is nobody to help me translate. As an example, could someone please help me with the statement "Fill out the courtesy check list first before performing repairs."

Who fills it?
What does it contain?
What I really don't understand is why must one fill a courtesy check list before doing a repair.

As a client you use a checklist to identify potential problem areas in your plumbing system, for instance. In that courtesy checklist you have to indicate any items you would like a technician to check during your appointment. It's important to fill out this form to ensure all plumbing problems (in this example) have been assessed and taken care of. Could be translated as: planilla de control para la atención al cliente.

You don't mention what kind of business you run. Anyway, you can tell them: "Por favor, complete la planilla de control antes de realizar cualquier reparación".

Now I have a clearer idea. What was confusing me is the use of the word 'courtesy'. Your proposed translation seems adequate: 'planilla de control para la atención al cliente'. The complete sentence would be: "Por favor llene la planilla de control para la atención del cliente antes de ejecutar las reparaciones".
