I think they say that so you will look at them and catch the ball, right?
I say "FOUR!!! (or FORE???)" because I am an avid golfer.

It´s a command to be alert or pay attention for something imminent that is going to happen.

According to Wiikipedia it is also a Poker term to play a single opponent. I have the impression that it was common argot in the military (perhaps especially the Navy (US) to pay attention to overhead pipes and other structures and generally to be aware of one's surroundings).

If heads-up is used as a noun or adjective, it should be hyphenated. e.g.: A heads-up team.
If heads up is used as a verb or interjection, it doesn't need to be hyphenated. e.g.: Heads up team! The player that heads up the team.

One way though to interpret heads up that we haven't mentioned here and that I heard twice last week is to alert rather as notify.
I wrote an email to a friend of mine and he repplied thanks for the heads up.
I also heard give me the heads up when you confirm it.
