Question Clarification

Hi everybody
I am translating a consent form for release of information . It is a one page format, and I want to know if I can mix English and Spanish or I have to just translate it in Spanish. What is the rule of translation for this? Thanks for your collaboration.

Ideally, it is better to have 1 release form in English and another one in Spanish. However, if that's not an option, you should consider printing something like this:

Name/Nombre:
Date of Birth/Fecha de nacimiento:

Etc.
