Default PCP and Referring Physician

When patients are being admitted to the hospital, 2 questions I need to ask them are:
  1. Do you have a PCP, or "Primary Care Physician"? (Family doctor)
  2. Did you have a "Referring Physician"? (Was there a doctor who sent you to this hospital?)
Can someone help me with these 2 expressions please?

I'm not sure about the second one, but I know that "Medico de cabecera" is used for "family doctor".

I don't know if your translation is intended for Spain or another Spanish speaking country.

In the USA Primary Care Doctor is often translated as 'médico de atención primaria'. The referring physician is the physician who refers patients to specialists. You can say 'médico que deriva' or 'médico derivador' (which sounds odd to me!).

The first one could be either, médico de familia o médico de cabecera, the second one is "quien lo refiere" although the question is just for a doctor, the referral can be from anothe patient

I also heard "Médico de Atención Primaria", mainly in US Spanish.

well, apparently i would never trust what you say because you are not familiar with the English language. a Traducer translates to :

Noun
1.traducer - one who attacks the reputation of another by slander or libelbackbiter, defamer, libeler, maligner, slanderer, vilifier
depreciator, detractor, disparager, knocker - one who disparages or belittles the worth of something
if you don't belive me look it up.

ok. so in this sense, I could be a traducer if I lied about your ability to
TRANSLATE. but, since I am correct, then you could say I dislike those who claim to know what they truly do not know.
In conclusion, if you are someone who translates things from one language to another, you are in fact a Translator, not a Traducer.

the answer to the question: referral translates to Remision in Spanish. unfortuantley, it is not the correct word you would use in this situation since most native speakrs, like myself, would not use this word. a more approprate world would be referencia. you could say: ?quien le hiso la referencia? or ?quien lo/la refirio?
