A very used possibility for "medical history" is "historia clínica".

I definitely agree with you Moni! 'Historia clínica' is also used in the USA.

I would go more for "antecedentes médicos" as "historia clínica" fits more with clinical records.

I was under the impression that in the US, clinical records is translated to: 'registros clínicos/médicos'. Things are never too clear when translating to the Spanish used in the US.
