Office of Vital Statistics & SSN
I am writing to you because I am translating a certification issued by the Florida Office of Vital Statistics -- DH FORM 1946 (08-04) -- into Spanish.
In that connection, I have a doubt about item 25 in relation to the use of "release". Is it used in the sense of "disclosing" or in the sense of "issuing"? In other words, does this office issue the Social Security Number? Or do parents request that it be issued at some other office and then this number is informed to the Office of Vital Statistics?
24. PARENT(S) REQUEST THAT A SOCIAL SECURITY NUMBER BE ISSUED FOR THIS CHILD (YES/NO)
25. PARENT(S) AUTHORIZE RELEASE OF CHILD'S SOCIAL SECURITY NUMBER TO THE OFFICE OF VITAL STATISTICS (YES/NO).
My attempts are as follows:
25. LOS PROGENITORES AUTORIZAN LA DIVULGACIÓN/LA EMISIÓN DEL NÚMERO DE SEGURIDAD SOCIAL DEL NIÑO AL REGISTRO CIVIL

Hey, I've check the SSA, at Your Social Security Number And Card
This is what I've learnt: <<How do I get my child a Social Security number? It is a good idea to get the number when your child is born. You can apply for a Social Security number for your baby when you apply for your baby’s birth certificate. The state agency that issues birth certificates will share your child’s information with us. We will mail the Social Security card to you.>>
So the correct translation would be: Divulgación.
Also, I'm not sure whether there's a difference, but I prefer "padres" to "progenitores".

"Padres" means the same as "progenitores", but I preferred the latter option because it sounds more formal.
