"Commando" means wearing no underwear. I don't know where this came from, but I have heard it!
(in fact there is even a episode of "Friends" in which this comes up )

I heard the expression "to freeball it" long before I heard "to go commando" for the first time, when a university professor asked a class of 700 for a show of hands of who wore boxers, who wore tighty-whities, and who went "commando."
