Default apocope -> apocopated form?

Hello everyone

Could you help me translate this phrase from the back of my high school's diploma?

La fotografia del alumno debe presentar las perforaciones marcando el APOCOPE de la institucion, la fecha de expedicion del certificado, asi como las siglas de la SUBDIRECCION encargada de la legalizacion del documento

This is my translation:

The student’s photo must have perforations marking the apocopated form of the institution, the certificate’s issuing date, as well as the acronym of deputy office which legalized the document.

My english/spanish dictionary gives me 'apocopated form' for 'apocope' but I'm not sure if this is correct because in my english dictionary I can't find anything related to 'apocopate'.

Also I'm not sure how to translate SUBDIRECCION, is 'deputy office' correct here? my dictionary also gives me 'post of deputy director'.
