The biggest thing I've had to get past as a young translator is wanting to do everything literally. I often find that the difficulties I have are of my own creation. The key is to relax and think about what the final translated product is going to read like.
Today I translated a date that was incorrectly written, given that it implied something happened to a patient eight days after he died. I left the incorrect date and added [sic], since I couldn't be certain which date had been intended.
Source typos occur often and are one more thing that make the translator's life difficult and should be corrected in the translation.
Regarding grammatical errors, DanaMarie's answer is a good one.
