In that courtesy checklist you have to indicate any items you would like a technician to check during your appointment. It's important to fill out this form to ensure all plumbing problems (in this example) have been assessed and taken care of. Could be translated as: planilla de control para la atención al cliente.

You don't mention what kind of business you run. Anyway, you can tell them: "Por favor, complete la planilla de control antes de realizar cualquier reparación".
